Children's Mental Health Case Management
Children's Mental Health Case Management Services at St. David's Center is for children (up to age 17) diagnosed with a Severe Emotional Disturbance (SED) who live in Hennepin County. To qualify, the child must receive assistance from the county in more than one area, such as special education, juvenile corrections, mental health services and medical assistance.
Our children's mental health case managers meet with families at least once a month to provide advocacy and support while also developing a care plan. This care plan could include therapy, special education services or changes in a child's school routine, mental health skills training, social skills building and family support.
We also can help families with:
- Health insurance issues, such as applying for Tefra
- School issues, such as arranging school meetings and advocating for an appropriate school program and individualized education plan (IEP)
- Referrals for treatment programs
- Referrals for individual and family therapy as well as medication management
- Referrals for mental health and/or personal care assistance (PCA)
- Locating appropriate skill building activities and services in the community
Our children's mental health case managers work flexible schedules that allow them to meet with you in your home, workplace or community.
Payment/Funding
All major insurance, Medical Assistance/TEFRA, PMAPs and private pay.
