At St. David’s Center, we are committed to supporting individuals with unique needs, by providing person-centered consultation services that support their right to make choices about the services that best meet their needs. Through this new opportunity, our team of caring, committed professionals will work with you to identify the service models, tools, and resources that will best serve your, or your loved one’s, personal goals and assessed needs.
Starting January 6th, we will begin scheduling appointments for individuals who were assessed in November for service agreements beginning January 1st. 2025.

Work with experienced professionals who understand the intricacies of Minnesota’s Community First Services and Supports (CFSS) program, ensuring you get the support you need.
Receive assistance in developing the CFSS Individual Service Delivery Plan tailored to your unique needs as identified in your assessment and your preferences for how those goods and services are delivered. Alternatively, you can develop your own plan for us to review.
Manage your own care if you prefer, through self-directed options. We provide the resources and tools to help you succeed including the risks and responsibilities of the two service models.
Gain clarity on eligible service options and interpreting your assessment in to the goods and services that support your independence and care needs.
Understand the responsibilities of caregivers and how to effectively manage care teams to ensure consistent, high-quality support. (Budget Model)
Access ongoing consultation and resources as your needs evolve, ensuring long-term success with CFSS services. We are here to support you as your needs change and can assist you with updates to your service delivery plan throughout the plan year.
Payment/cost: The Initial 12 months will include two service authorization periods. Your initial 120-day PCA/CFSS service authorization will include 6 sessions. Following the 120-day period you will receive a 6-month CFSS service authorization with 6 new sessions of Consultation Services. Consultation Services are not a part of your budget and do not decrease your assessed allocation.
